You can find us on the second floor of the Midland County Services Building at 220 W. Ellsworth Street near downtown Midland (four story brown brick building).
When you enter the building, proceed to the second floor (elevators and stairs available).
We have two separate check-in areas, depending on the services you are seeking. These areas can be located using the room number (located on the top of the doorframe) or signage:
-Clinic Services- Room 231
-Administration, Community Health, and Environmental Health- Room 265
