Guardianship


  1. Ensure that ward is in adequate housing environment. Arrange for safe and appropriate housing if necessary.
  2. Ensure that ward is medically stable and that all medical needs are being addressed.
  3. Ensure that ward’s basic needs are met, such as having weather appropriate clothing, food, and shelter. Assist ward with shopping, accessing food banks, etc. if necessary.
  4. Correspond with doctors, hospitals, and other medical professionals regarding all medical issues, including medical emergencies.
  5. Attend all care conferences/planning meetings regarding ward to have input in their care.
  6. Apply for and maintain benefits from various agencies, such as Social Security, Medicare, Medicaid, housing, food stamps, independent insurance companies, and other social/community programs as necessary.
  7. Arrange for schooling and job training/placement if applicable.
  8. Make/receive phone calls pertaining to ward’s care. This may include speaking with the ward, family members, community supports, placement agencies, residences, etc.
  9. Make copies and mail documents pertaining to ward’s care.
  10. Maintain records of ward’s care, including health & housing information.
  11. Visit the ward a minimum of once every three months (4 times per year). Maintain records of dates, duration and subject matter of each visit.
  12. Work with other agencies for ward’s care such as Community Mental Health, the ARC, volunteer agencies, nursing homes, etc.
  13. Appear at any legal proceedings that affect the ward.
  14. Prepare and file all reports required by the court.
  15. Provide additional services as needed.
